These forms must be properly completed and forwarded to the Board to be considered for a license to practice chiropractic in the state of North Carolina. Forms can be downloaded, then filled out and mailed to:

All applications must be accompanied by a photo.

Requirements for Applicant's Photograph

The photograph you are to submit will be used during the examining process. It should be approximately 2" x 3" and of recent, reasonable likeness.

The photo must be signed by the applicant and the signature notarized with the notary stamp or seal on the back.

This photograph must be received at the time of the application.
