Incorporation Forms
For your convenience, the Board provides forms necessary to form a corporation in the state of North Carolina.
When the Certificate of Incorporator and Application
for Certificate of Registration have been executed, please return them to
the Board office, along with four (4) copies of the Certification by the
Board of Chiropractic Examiners of the State of North Carolina. We will
execute the Certification and return three (3) copies to you. The original
should then be filed with the Secretary of State.
A copy of the Articles of Incorporation and a check in the amount of $50.00 for certification fee, made payable to the N.C. Board of Chiropractic Examiners, must accompany the application.
